How do I know if I am eligible for benefits?

If you work an average of 30 or more hours per week, you are eligible for medical, dental and vision benefits. You must work an average of 35 or more hours per week to be eligible for most other benefits, such as disability, Supplemental Life and AD&D. Please see your Benefits Guide for more details on eligibility.

What if I have questions about my benefit options?

We know that you may have questions about your benefits. That’s why the Retail Benefits Helpline is here to help you! The Retail Benefits Helpline offers you a variety of services. The benefits helpline will help you:

  • Get answers to your questions about benefits and Open Enrollment
  • Find a network doctor using the Provider Finder®
  • Order medical ID cards
  • Confirm eligibility
How do I learn about my benefit options?

We have created a variety of communication materials to help you learn about the benefit programs. This site is a great online resource. In addition to these FAQs, you'll find other helpful information here like a learning video, a recording of the enrollment meeting, the Medical Match Up interactive feature that can help you see which medical plan could be right for you and your family.

Who is the Medical Plan claim administrator?

Collective Health is the medical plan claims administrator. You will continue to utilize the BCBS network of providers. When you have a question, need to file a claim, or search for a doctor, you will contact Collective Health. Representatives will answer your questions and assist you as needed.
